Informed consent for Energy 2 Energy services
Please read the complete terms before electronically signing while requesting your first appointment.
Nature of services
Energy 2 Energy provides complementary energy-balancing and educational services, including Emotion Code, Body Code, Belief Code, and related reflective practices. These services are intended to support personal awareness and well-being. They are not medical care, mental-health care, diagnosis, treatment, or a substitute for services from licensed health professionals.
No guarantees
Responses to energy work vary. No outcome, improvement, cure, or specific result is promised or guaranteed. I understand that I remain responsible for my decisions, health care, and use of any information or practices discussed.
Professional care and emergencies
I will not delay, discontinue, or replace medical, veterinary, psychological, psychiatric, or other licensed professional care because of an Energy 2 Energy session. Energy 2 Energy does not provide emergency services. For an urgent medical or mental-health concern, I will contact an appropriate professional or emergency service.
Remote sessions and technology
Sessions may occur remotely by telephone or video meeting. I understand that internet and communication services may experience interruptions. I will choose a reasonably private location and understand that Energy 2 Energy cannot control the privacy of my surroundings or devices.
Session records and client portal
I authorize Energy 2 Energy to maintain my appointment information, signed consent, client-visible session summaries, private practitioner notes, messages, and assigned resources in a protected client system. Client-visible notes and resources may be shared with me through my portal. Private practitioner notes are not displayed in the client portal.
Electronic communication
I understand that portal messages and email may be used for scheduling, administrative communication, educational resources, and session follow-up. Email is not appropriate for urgent or emergency communication.
Voluntary participation
My participation is voluntary. I may ask questions, decline a technique, pause a session, or stop services at any time. I understand that fees, cancellations, refunds, and rescheduling are governed by the policies presented when I book or pay.
Animal sessions
For services involving an animal, I confirm that I am the animal owner or have authority to request the session. Energy work does not replace veterinary examination, diagnosis, treatment, or emergency care.
Acknowledgment and electronic signature
By typing my legal name and electronic signature and selecting the agreement checkbox, I confirm that I have read this consent, understand it, have had the opportunity to ask questions, am at least 18 years old or otherwise authorized to consent, and voluntarily agree to receive Energy 2 Energy services.
